I authorize OPTIMAL Graphics and Business Services to withdrawl my monthly payment for services from the account I have provided above on a monthly basis. I understand this authorization will remain in effect until I give verbal or written notification to terminate this service. If I wish to cancel the automatic payments and return to receiving a monthly invoice for services, I may do so at any time, given a 24 hour notice is made prior to the next withdraw date of my payment, by calling the office at (832) 578-1474.
